Horizon Health Commisioning
Founded in 2006, Horizon Health Commissioning represents the 26 GP practices in North Bedfordshire with a combined patient list size of over 160,000. We rely on the energy and passion that our practices have for patient care and the improvement of clinical standards. Based on the knowledge of the needs of the people registered at our practices, we believe we are in the best position to assess and redesign the services and patient pathways to ensure the ultimate in medical care.
Horizon Health Commissioning draws upon the expertise of its members to undertake this service redesign and so offer you these improved services and make cost efficiencies which can be reinvested in other improvements and new services to improve all our health. We place great emphasis upon partnership working to ensure that patients, where necessary, can move between services and providers with ease.
We enjoy excellent working relations with local NHS organisations such as Bedford Hospital Trust and Bedfordshire Primary Care Trust which creates an atmosphere where improvement flourishes. We operate cohesively with our neighbouring Practice Based Commissioners to share ideas and strategy for patient services that transcend our boundaries. Our clinicians lead service change alongside their colleagues, such as Hospital Consultants, to ensure that each pathway is centred on the patient throughout their care.
